Steven Kopas and Nancy Lightheart, Contingent Managers, need to collect some personal and health information from each athlete so they can better serve the needs of the Contingent members.

This form also contains a release allowing team management to seek medical treatment for your child in the event of an emergency.

The attached Athlete Information Sheet must be completed and returned to Joanne Moser, Contingent Coordinator at the Contingent Meeting on May 22.
