These membership forms should only be used by people living in a province that does not have a CBTF-affiliated provincial organizing body (Nova Scotia, Prince Edward Island, Newfoundland).

In provinces with a CBTF-affiliated provincial organizing body (links to the provincial websites), all membership applications (or renewals) should be submitted through the provincial organization using provincially-approved forms and fees.
