The Brochure and Entry Form for the 2011 International Cup Qualifier is attached.

A few details to note:

1. The entry deadline is April 15, 2011. All entries must be received by that date.
2. The Declaration to compete that was submitted earlier does not take the place of the competition entry form. You must still complete and submit the entry form in order to compete at the Qualifier in Calgary.
3. Athletes that did not submit a Declaration by the October 31 deadline, but that are interested in competing for a spot on the Contingent may do so. In addition to the Qualifier Entry Form, these athletes must also submit their IC Declaration [1], their Declaration Deposit ($200), and a Late Declaration Fee ($25).
