As part of the athlete accreditation process, the WBTF requires that each athlete provides a Health Certificate indicating they are in generally good health and is fit to compete.

The official CBTF Health Certificate is attached. It must be completed and signed by a physician. The original should be returned to Steve Kopas, Contingent Manager, no later than June 20.
