These are sample membership forms that can be used as a template by a provincial organization (or clubs, with approval from their provincial organization). These are templates only - they must be modified to include the provincial or club organization, and be updated to reflect the correct fees for that province. English and French form templates are attached below.

In provinces with a CBTF-affiliated provincial organizing body [1], all membership applications (or renewals) must be submitted using the provincially-approved form and fees, and should be submitted and processed through a local club or the provincial registrar.

For those living in a province that does not have a CBTF-affiliated provincial organizing body (Nova Scotia, Prince Edward Island, Newfoundland), 'Direct Membership' option is available. Individuals should contact the CBTF National Registrar (2ndvpcbtf [dot] ca) directly for the appropriate membership form and fees.
