The CBTF Contingent Hotel Worksheet and Payment Forms are now available.

A document and information checklist is also included.

Questions about the Contingent Package or the Package Payment Forms should be directed to the WC Contingent Coordinator at officecbtf [dot] ca.

All forms (Hotel & Payment Worksheets, Document Checklist, Athlete Information Sheet [1], Code of Conduct [2], and Secret Pal [3]), and the initial deposit will be collected at the Contingent Meeting at the conclusion of the 2022 Canadian Team Trials. The Health Certificate [4] should have been sumitted with your Team Trials entry form, but if a revised version is required it will be collected at that time also.

We remind travelers that if their child is under the age of majority, a travel consent form (if travelling with no parents or one parent) and a chaperone form (if travelling with no parents) are required. These are available on request (officecbtf [dot] ca) if required.
