The Contingent Manager collects some personal and health information from all athletes to better serve the needs of the Contingent members.

This form contains a release allowing team management to seek medical treatment for your child in the event of an emergency.

The attached Athlete Information Sheet must be completed and can be submitted online in advance of March 11th to officecbtf [dot] ca, or submitted in person at the Contingent Clinic.
